1. Contact our Customer Service Group by phone at (847) 478-2500 or fax at (847) 478-2284 for a Repair Return Authorization Form, or download them here.      
2. Fill the appropriate form out with a detailed description of the problem and pertinent machine information, and fax it back to (847) 478-2503.        
3. The Customer Service Group will fax back the Return Authorization Number, along with the price for the repair.        
4. Sign the Authorization Form, put a copy in the box with the unit, and ship to Mitsubishi Electric Automation, Inc., Attn: Repair Department, 500 Corporate Woods Parkway, Vernon Hills, IL 60061. Please note the Return Authorization Number on the outside of the box.        
5. The unit is received into the Logistics Group, logged into our Enterprise Database, and sent to repair.        
6. Once received into the Repair Department, your unit is ultrasonically cleaned and routed to the appropriate Repair Engineer Group for testing.        
7. Our Repair Engineers will review the claim information you have provided, and perform the appropriate static and dynamic tests to determine the cause of failure.        
8. Once the troubleshooting phase is complete, our Repair Engineers repair the unit by replacing the affected components. Our Repair Engineers document the cause and repair information in our Enterprise Database system.        
9. The Repair Engineers then re-test the unit and allow it to run for a specified period of time to ensure that the unit has been fully repaired.        
10. A Repair Letter is generated, which displays pertinent information about the unit, including the work that was performed by the Repair Engineers. This letter is placed in the box with your unit for your records.        
11. Your unit is then routed back through Logistics for return shipment to you.
